Health Forward Foundation Grant Application Guidelines
To apply for a grant from the Health Forward Foundation, please ensure your organization meets the following eligibility requirements and complete the application in its entirety via the online form here.
Eligibility Requirements
- The applicant organization must have a medical/Health Care/Health Education purpose.
- The organization must be tax-exempt under Section 501(c)(3) of the Internal Revenue Code.
- A copy of the IRS tax-exempt determination letter must be included with the application.
Eligible Project Types
- Grants are available for educational purposes, medical training equipment and supplies, or other projects aimed at enhancing medical services/education within the community.
Restrictions
- Grants are not provided for religious purposes or to government agencies.
- Individuals are not eligible for financial assistance or scholarships.
- The Foundation does not fund operating expenses or endowment funds.
Geographical Focus
- Funding is currently restricted to projects benefiting the Greater Utica and Mohawk Valley regions.
Board Support
- A letter of support from the applicant organization’s Board of Directors is required.
Evaluation and Reporting
- Applicants must outline how the project will be evaluated, including how beneficiaries are helped, outcomes achieved, and the project’s impact.
- Past grant recipients must report how previous funds were used and who benefited.
Submission Requirements
- Complete the official grant application here, including the cover sheet, proposal, project budget, and audited financials (if available).
- Submit the completed application by 11:59 PM, March 31, 2025.
Publicity Commitment
If awarded a grant, recipients must provide timely publicity about the funding and share copies of printed materials and social media announcements with the Foundation.

The deadline for grant applications is 11:59 PM, March 31, 2025.
